Yellowknife residents can now request vaccination records in person at two locations in the city, the NWT’s health authority announced.
The authority said people can go to either the Jan Stirling Centre or Stanton Territorial Hospital Mondays through Fridays to request the vaccination records. The health authority said offices are open from 8:30am to 4:30pm but closed for lunch from 12-1pm.
You will be asked to fill out a form to obtain your personal records or records on someone’s behalf. You need to provide a piece of valid photo government identification or two pieces of valid non-photo identification, the health authority said in a Tuesday news release.
“Visiting either of these sites in person will enable clients to receive copies of their vaccination records more efficiently than by email or by calling ahead of time. This includes receiving records during their visit or on the same day,” the authority stated.
The authority said the additional service was being offered as a “streamlined approach to obtaining vaccination records as the 2026-2027 school year approaches.”
If you received vaccination in Yellowknife but cannot go in person to request your vaccination records, the health authority said you can continue to request vaccination records by emailing the authority with the corresponding form and identification.
For those vaccinated outside Yellowknife, contact your community’s health centre or public health unit for your records. If you were vaccinated outside the NWT, contact the public health unit or healthcare provider in that jurisdiction.
